ACH AUTHORIZATION REQUIRED
A receipt for each payment will be emailed to you and the charge will appear on your bank statement as an "ACH Debit." You agree that no prior-notification will be provided unless the date or amount changes, in which case you will provide notice by 2PM on Wednesday.
Disclosure for Authorization to Bill
By completing the enrollment process, I authorize Childs Foundation (GoToWork) to charge my credit card/debit card indicated below for a weekly services. A one time membership fee of $15.00 will be charged immediately to complete registration in addition to the first week of services.
