Ach Authorization

A receipt for each payment will be emailed to you and the charge will appear on your bank statement as an “ACH Debit.”  You agree that no prior-notification will be provided unless the date or amount changes, in which case you will provide notice by 2pm on Wednesday.

Disclosure of Authorization to Bill

I authorize Childs Foundation (Go To Work) to charge my credit card/debit card indicated below for a weekly services. A one time membership fee of $15.00 will be charged immediately to complete registration in addition to the first week of service.
